Every effort is made to accommodate your needs. While in most instances we can accommodate you quickly. Please to be sure to tell the staff any special needs you may have in relation to your visit.
What to bring to your first visit
Any relevant information on your condition including doctors notes, X-Ray, CT or MRI films or disks, any information on medicines you take.
All of your insurance cards- primary, secondary and any supplemental information that will be useful to the office staff.
Payment for services rendered after your visit. Please note if you require additional tests such as X-rays, this will be charged separately by the hospital’s radiology department.
Insurance Participation
Dr. Wolfe participates in several insurance plans at both office locations. When you call for an appointment, please have your primary insurance plan Card at your hand so we can help you understand your benefits. Even if Dr. Wolfe does not participate with your plan, chances are you can still come to see him out-of-network. Our staff is highly trained to assist you with this and can address any questions you may have. We are here to help you and we do not want insurance to stand in the way.
If you need to cancel an appointment

that we can offer your appointment time to patients on our waiting list.
We
recognize that your time is valuable, and we make every effort to run
on time. Occasionally emergencies or patients require a little more
time, and these cause scheduling delays beyond our control. We
apologize if we keep you waiting.
